Select the program you would like to establish in your service and complete your details below. Payment methods include ‘Pay by Invoice’.

  • Facilitator Details

    The Facilitator is the person who will oversee the program in your service. An account will be created for them to access the online portal. They will be able to view the portal once payment is completed.

  • This is the name you wish to give your group. E.g. GenGiggles
  • The date you plan to start your program. You are able to change this later if you need to.
    Select the day/s your session will take place. You can change the day later if needed.

  • This description will be displayed on the Find a Playgroup search page.

  • Blue Card Details

    All Facilitators are required to have or obtain a Blue Card. Ageless Play can assist with applications. If you already have the Blue Card Information for your Facilitator please complete it below. Otherwise, you will need to update it before your program commences.
  • For insurance purposes, Ageless Play and our insurers must be notified if your session will be participating in excursions or in-water activities during your playgroup session. This includes the use of private home pools during your session. Please contact Ageless Play to request the appropriate insurance forms. By ticking this box you agree to comply with this.
  • I confirm that the information in this form is, to the best of my knowledge, true and correct. I acknowledge that information about Ageless Play Sessions and personal information from families and services is collected for the purposes of referring families to playgroups and in accordance with government funding requirements and to assist Ageless Play in its planning and service delivery.
